Welcome to the Ahsi Aesthetics by Rome Health (the “Practice”). These terms and conditions govern the Practice’s Monthly Membership Program (the “Program”). By enrolling in this Program, you agree to the following legally binding terms and conditions of your membership. Please read them carefully.

  1. 1. Membership Eligibility

    • Membership is available to individuals 18 years or older.
    • Membership is non-transferable and can only be used by the enrolled member.
    • A valid credit card or debit card and email address is required for enrollment in the Program. Members must complete the Credit Card Authorization Form.
  2. 2. Membership Fees & Payment

    • The monthly membership fee is $199 and will be automatically charged each month, in advance, to the payment method on file until your membership is terminated in accordance with these terms. Your obligation to pay membership fees is not dependent upon your usage of the Program.
    • Membership fees are non-refundable and subject to change with 30-day notice.
    • You are responsible to ensure the credit/debit card on file is valid so the monthly membership fee can be charged to that card.
    • If a payment is declined, the membership benefits will be suspended until payment is successfully processed. Your membership in the Program must be in current (paid) status to utilize the full benefits of your membership.
  3. 3. Membership Benefits

    • Members will receive the following benefits:
      • 10% off laser treatments
      • 10% off chemical peels
      • 10% off of skincare line products
      • Neurotoxin at $14/unit
      • $100 off facial fillers (Radiesse and Revanesse Versa)
    • Discounts are valid only on full-price products and services. Discounts cannot be combined with other promotions unless explicitly stated. Member has the option to choose which offer would be most beneficial.
    • Benefits apply only to services and products purchased during an active membership period.
    • Unused benefits do not roll over to the next month.
    • Fees for treatments or services outside of the Program will be charged separately at the Practice’s standard rates, unless otherwise stated.
  4. 4. Appointment Policies

    • Members must schedule appointments in advance, which appointments are subject to availability. Your membership in the Program does not include special appointment setting or scheduling, or guarantee that a particular individual will be available to administer services.
    • A 24-hour cancellation policy applies. No-shows or late cancellations may result in a forfeited service fee. [If the 24-hour cancellation policy triggers fees, the fees will need to be paid before you may schedule another appointment.]
    • Membership discounts do not apply to missed or canceled appointments.
    • Late arrivals may result in reduced treatment time or rescheduling.
  5. 5. Membership Cancellation

    • Members may cancel their membership at any time upon 30-days’ advance notice via email or in person.
    • Requests for cancellations must be completed before the next billing cycle to avoid being charged for the following month.
    • Upon cancellation, you will retain access to Membership benefits until the end of the current billing cycle.
    • There are no refunds for partial months or unused benefits.
  6. 6. Practice’s Right to Terminate Membership

    • Ahsi Aesthetics by Rome Health reserves the right to terminate or suspend a membership at any time due to:
      • Non-payment
      • Violation of policies
      • Abusive or inappropriate behavior toward staff or other clients
      • Any other reason at the discretion of management
    • If the Practice cancels the membership for reasons other than non-compliance with these terms and conditions, a pro-rated refund may be issued.
  7. 7. Modifications to Terms

    • Ahsi Aesthetics by Rome Health reserves the right to unilaterally add, remove or modify membership terms, pricing, services or benefits with a 30-day notice. Any changes to these terms and conditions will be communicated to you via email.
    • Continued use of your membership after changes to the terms and conditions are implemented constitutes agreement to the modified terms, pricing or benefits.
  8. 8. Liability & Medical Disclaimer

    • Membership in the Program does not replace professional medical advice, and questions, concerns, and treatment decisions should be discussed in advance with your regular healthcare provider as appropriate.
    • Members assume full responsibility for their treatment decisions. You are required to disclose any relevant health conditions, medications, allergies, or treatments that may affect the safety or effectiveness of the treatments provided. You are responsible for ensuring that you meet the eligibility criteria for each treatment.
  9. 9. Governing Law

    • These terms and conditions are governed by the laws of the State of New York, and any disputes will be resolved in the applicable jurisdiction. If any of these terms and conditions are found to be invalid or unenforceable, the remainder will be valid and enforceable to the fullest extent of the law.
  10. 10. Electronic Communications

    • Communications between you and the Program use electronic means, including communications via email. You consent to receive communications regarding the Program and these terms and conditions in electronic form and agree that all terms, conditions, agreements, notices, disclosures and other communications that are provided to you electronically satisfy any legal obligations that such communications would satisfy if provided in hard copy writing.
